The baseline eye examinations are performed at the Mount Auburn Hospital, Occupational Health Services, located at 725 Concord Avenue, Suite 5100, Cambridge, MA 02138 (map)

The following information is provided for your assistance in arranging for an eye exam. The individual department or laboratory is responsible for all fees and costs associated with the baseline or post injury eye exam. This fee is currently $62 per person and employees will not need to pay anything at the time of the visit; Tufts EHS will pay for the initial visit and will bill the departments separately. Note if there are medical findings, the cost of the exam can increase.

1. Contact Mount Auburn Hospital by dialing 617-354-0546 to schedule the examination.
2. Inform the representative that:
   a. You are from Tufts University and are part of the “Tufts Laser Vision Environmental Group”
   b. You work in a laser laboratory and a baseline eye examination is recommended.
   c. You will need a copy of your examination results for the purpose of your own record.
3. Anticipate a temporary impaired vision after the examination.
