Tufts University Forms

The Office of Research Administration (ORA) is responsible for all pre-award phases of grant applications. In order to meet federal requirements, all grant applications, along with university-required forms, are reviewed by ORA before submission to the funding agency. Some forms apply university-wide, others are used only for the Health Sciences Campus, and others only for the Medford/Somerville Campus. Each category is detailed below. Those applying for grants must fill out the specific forms required for the application before submission to ORA for review. Not all applications require all forms, but some forms (e.g., internal sponsored coordination form, by campus) are required on all applications.
